CVS Health Opens New Pharmacy and Senior Care Center in Erie
CVS Health has unveiled a new integrated health care center in Erie that pairs a full-service CVS Pharmacy with an Oak Street Health primary care clinic for Medicare-eligible adults.
This new model is part of a rollout that also includes locations in Harrisburg and Norristown, aimed at providing more accessible, personalized care for older adults.
Located at 1535 West 26th Street, the Erie facility features a side-by-side layout that connects pharmacy services with value-based primary care, including behavioral health, social support, and Medicare education.
The approach is designed to remove barriers, coordinate care, and improve health outcomes, particularly for patients managing chronic conditions.
CVS pharmacists will work directly with Oak Street Health providers to help patients manage medications, access cost-saving options, and receive timely vaccinations.
Oak Street Health’s care model emphasizes long-term relationships with providers, a key factor in promoting healthier outcomes for older patients.
The CVS Pharmacy remains open to the public, while Oak Street Health accepts all Medicare forms, including Original Medicare, Medicare Advantage, and dual-eligible plans.
